The Hospital for Sick Children and RAND, a nonprofit, nonpartisan research organization, are conducting a study to identify quality indicators that can help measure and track outcomes for transition from paediatric to adult care.
Quality Indicators for Transition from Paediatric to Adult Care for Youth with Chronic Conditions: A Consensus-Building Multistakeholder Initiative
This multi-stage study earlier surveyed youth currently transitioning from paediatric to adult care or who had completed such a transition; parents, guardians, or caregivers of such youth; clinicians involved in transition care delivery; and healthcare administrators involved in child health management, leadership, or administration.
The next stage involves a consensus-building process in which experts take the existing data and work together to create a set of quality indicators for the transition to adult care that apply across physical, developmental, and mental health conditions, clinical care settings, and health jurisdictions.
